Pragmatic Free Trial Meta
Pragmatic Free Trial Meta is a non-commercial, open data platform and infrastructure that facilitates research on pragmatic trials. It collects and distributes clean trial data, ratings and evaluations using PRECIS-2. This allows for a variety of meta-epidemiological analyses to examine the effect of treatment across trials of different levels of pragmatism.
Background
Pragmatic studies are increasingly acknowledged as providing evidence from the real world for clinical decision making. The term "pragmatic", however, is used inconsistently and its definition and measurement need further clarification. Pragmatic trials must be designed to inform policy and clinical practice decisions, not to confirm an hypothesis that is based on a clinical or physiological basis. A pragmatic study should strive to be as close to the real-world clinical environment as is possible, including its recruitment of participants, setting up and design of the intervention, its delivery and implementation of the intervention, and the determination and analysis of the outcomes, and primary analysis. This is a significant distinction from explanatory trials (as described by Schwartz and Lellouch1) that are intended to provide a more thorough proof of a hypothesis.
The trials that are truly pragmatic must not attempt to blind participants or the clinicians, as this may cause bias in the estimation of the effect of treatment. The trials that are pragmatic should also try to attract patients from a wide range of health care settings, to ensure that the results can be compared to the real world.
Finally studies that are pragmatic should focus on outcomes that are vital to patients, such as quality of life or functional recovery. This is particularly important when trials involve the use of invasive procedures or could have dangerous adverse effects. The CRASH trial29, for example, focused on functional outcomes to compare a 2-page case-report with an electronic system for monitoring of patients in hospitals suffering from chronic heart failure, and the catheter trial28 used symptomatic catheter-associated urinary tract infections as the primary outcome.
In addition to these aspects the pragmatic trial should also reduce the trial procedures and data collection requirements to reduce costs. Furthermore, pragmatic trials should seek to make their findings as applicable to clinical practice as possible by making sure that their primary analysis is the intention-to-treat approach (as described in CONSORT extensions for pragmatic trials).
Many RCTs which do not meet the criteria for pragmatism, but have features that are in opposition to pragmatism, have been published in journals of different types and incorrectly labeled pragmatic. This can lead to false claims about pragmatism, and the term's use should be made more uniform. The creation of a PRECIS-2 tool that offers an objective and standardized assessment of pragmatic features is a first step.
Methods

In a pragmatic research study the aim is to inform clinical or policy decisions by showing how an intervention can be integrated into routine care in real-world contexts. Explanatory trials test hypotheses regarding the cause-effect relation within idealized settings. In this way, pragmatic trials could have lower internal validity than studies that explain and are more susceptible to biases in their design, analysis, and conduct. Despite their limitations, pragmatic studies can be a valuable source of information for decision-making within the healthcare context.
The PRECIS-2 tool evaluates an RCT on 9 domains, with scores ranging between 1 and 5 (very pragmatic). In this study, the recruitment, organisation, flexibility: delivery, flexible adherence and follow-up domains were awarded high scores, but the primary outcome and the procedure for missing data fell below the limit of practicality. This suggests that a trial could be designed with effective practical features, yet not damaging the quality.
It is, however, difficult to determine the degree of pragmatism a trial really is because pragmatism is not a binary characteristic; certain aspects of a trial may be more pragmatic than others. Moreover, protocol or logistic changes during an experiment can alter its pragmatism score. Results
While the definition of pragmatism does not require that all trials be 100% pragmatic, there are advantages of including pragmatic elements in clinical trials. These include:
Increased sensitivity to real-world issues which reduces cost and size of the study, and enabling the trial results to be more quickly transferred into real-world clinical practice (by including patients who are routinely treated). However, pragmatic studies can also have disadvantages. The right amount of heterogeneity, for example could allow a study to generalise its findings to many different patients or settings. However, the wrong type can reduce the sensitivity of an assay, and therefore decrease the ability of a study to detect small treatment effects.
A variety of studies have attempted to categorize pragmatic trials using various definitions and scoring methods. Schwartz and Lellouch1 have developed an approach to distinguish between research studies that prove the clinical or physiological hypothesis, and pragmatic trials that inform the selection of appropriate therapies in the real-world clinical setting. The framework was comprised of nine domains evaluated on a scale of 1-5 which indicated that 1 was more informative and 5 was more practical. The domains covered recruitment of intervention, setting up, delivery of intervention, flexible compliance and primary analysis.
The original PRECIS tool3 was based on a similar scale and domains. Koppenaal and colleagues10 created an adaptation of this assessment, known as the Pragmascope that was simpler to use for systematic reviews. They found that pragmatic systematic reviews had higher average scores across all domains, but lower scores in the primary analysis domain.
This difference in the analysis domain that is primary could be explained by the fact that the majority of pragmatic trials analyse their data in the intention to treat way while some explanation trials do not. The overall score for pragmatic systematic reviews was lower when the domains of management, flexible delivery and follow-up were merged.
It is important to understand that a pragmatic trial does not necessarily mean a low-quality trial, and in fact there is an increasing rate of clinical trials (as defined by MEDLINE search, but this is neither sensitive nor specific) that use the term "pragmatic" in their abstracts or titles. The use of these terms in abstracts and titles may suggest a greater awareness of the importance of pragmatism, but it is unclear whether this is manifested in the content of the articles.
Conclusions
As the importance of real-world evidence grows commonplace the pragmatic trial has gained traction in research. They are randomized clinical trials that compare real-world care alternatives instead of experimental treatments in development, they involve patient populations that are more similar to those treated in routine care, they employ comparisons that are commonplace in practice (e.g., existing drugs) and depend on participants' self-reports of outcomes. This method can help overcome the limitations of observational research which include the biases that arise from relying on volunteers, and the limited availability and coding variability in national registry systems.
Other advantages of pragmatic trials include the ability to use existing data sources, and a higher probability of detecting significant changes than traditional trials. However, these tests could have some limitations that limit their reliability and generalizability. Participation rates in some trials may be lower than anticipated due to the health-promoting effect, financial incentives, or competition from other research studies. Many pragmatic trials are also restricted by the necessity to enroll participants on time. Certain pragmatic trials lack controls to ensure that observed variations aren't due to biases during the trial.
The authors of the Pragmatic Free Trial Meta identified 48 RCTs that self-labeled themselves as pragmatic and that were published until 2022. They assessed pragmatism using the PRECIS-2 tool that includes the domains eligibility criteria and recruitment criteria, as well as flexibility in intervention adherence, and follow-up. They found that 14 of these trials scored as highly or pragmatic pragmatic (i.e. scores of 5 or higher) in one or more of these domains, and that the majority of these were single-center.
Studies with high pragmatism scores tend to have broader criteria for eligibility than traditional RCTs. They also have populations from many different hospitals. The authors argue that these characteristics could make the pragmatic trials more relevant and useful for everyday clinical practice, however they do not guarantee that a trial conducted in a pragmatic manner is free of bias. The pragmatism principle is not a fixed characteristic the test that doesn't have all the characteristics of an explanation study may still yield valuable and valid results.
